Biomarcadores preditivos de resposta são essenciais para orientar efetivamente o tratamento direcionado do câncer. A ataxia telangiectasia e os inibidores de quinase relacionados a Rad3 (ATRi) demonstraram ser letais sintéticos com perda de função (LOF) da quinase ataxia telangiectasia-mutada (ATM), e estudos pré-clínicos identificaram alterações sensibilizadoras de ATRi em outras respostas a danos no DNA ( genes DDR). Aqui, relatamos os resultados do módulo 1 de um estudo de fase 1 em andamento do ATRi camonsertib (RP-3500) em 120 pacientes com tumores sólidos avançados com alterações LOF nos genes DDR, previstos por telas CRISPR quimiogenômicas para sensibilizar tumores ao ATRi. Os objetivos primários foram determinar a segurança e propor uma dose recomendada de fase 2 (RP2D). Os objetivos secundários foram avaliar a atividade antitumoral preliminar, caracterizar a farmacocinética do camonsertibe e sua relação com biomarcadores farmacodinâmicos e avaliar métodos para detectar biomarcadores sensibilizadores de ATRi. Camonsertib foi bem tolerado; a anemia foi a toxicidade relacionada ao medicamento mais comum (32% grau 3). O RP2D preliminar foi de 160 mg semanalmente nos dias 1–3. A resposta clínica geral, o benefício clínico e as taxas de resposta molecular em subtipos tumorais e moleculares em pacientes que receberam doses biologicamente eficazes de camonsertibe (>100 mg d−1) foram de 13% (13/99), 43% (43/99) e 43 % (27/63), respectivamente. O benefício clínico foi maior no câncer de ovário, em tumores com alterações bialélicas LOF e em pacientes com respostas moleculares. Registro ClinicalTrials.gov: NCT04497116.
A resposta ao dano do DNA (DDR) é indispensável para a manutenção da integridade genômica e sobrevivência celular. A perda de componentes específicos da maquinaria DDR resulta em distintas formas de instabilidade genômica1. A ataxia telangiectasia e a quinase relacionada a Rad3 (ATR) desempenham um papel integral na DDR, desencadeando uma cascata de eventos em resposta a danos no DNA e estresse de replicação2,3. Visar defeitos DDR por meio de letalidade sintética é uma abordagem clinicamente validada para o tratamento do câncer4,5,6. Essa abordagem é exemplificada pelos inibidores da poliadenosina difosfato-ribose polimerase (PARP), que receberam aprovação regulatória para tratar pacientes com vários tipos de tumor com mutações de perda de função (LOF) BRCA1 ou BRCA2 (BRCA1/2) e outras alterações selecionadas em diferentes configurações.
Em estudos pré-clínicos e clínicos iniciais, a inibição de ATR demonstrou ser sinteticamente letal com LOF da quinase ataxia telangiectasia-mutated (ATM)7,8. Embora os primeiros estudos clínicos que investigam a inibição de ATR em tumores que abrigam mutações ATM ou falta de expressão da proteína ATM tenham mostrado sinais preliminares de atividade antitumoral, o método ideal para identificar ATM LOF em uma população mais ampla ainda precisa ser estabelecido. Nossa hipótese é que o diagnóstico e o tratamento precisos dos tumores ATM LOF requerem a determinação do estado alélico (bialélico versus não bialélico) e a exclusão de alterações ATM LOF decorrentes da hematopoiese clonal. Além disso, levantamos a hipótese de que a inibição do ATR resulta em atividade antitumoral em alterações DDR além do ATM, como BRCA1/2 e outros. Especificamente, a atividade clínica da inibição de ATR em tumores resistentes ao inibidor de PARP (PARPi), incluindo cânceres com mutações de reversão BRCA1/2, não foi relatada. Nós investigamos se a inibição do ATR poderia ser benéfica para esses pacientes e em outras áreas críticas de necessidades clínicas não atendidas.
Alterações cancerígenas sensibilizadoras de múltiplos inibidores de ATR (ATRi) foram propostas por meio de triagem quimiogenômica direta habilitada por interferência de RNA ou CRISPR-Cas99,10,11,12,13. Usamos esses conjuntos de dados de tela habilitados para CRISPR quimiogenômico, juntamente com dados de validação pré-clínica internos e publicados, para identificar alterações DDR sensibilizadoras de ATRi como a base racional para a seleção de pacientes para tratamento com camonsertib (RP-3500) (Métodos e Fig. 1a)10 ,13,14,15,16,17,18,19.
